Transitioning
Assists people in a long term care facility who want to live independently in the community through Nursing Home Transition. This may include locating housing and support services and teaching daily living skills. Transitioning is also provided to consumers in the Community Services Program for Persons with Physical Disabilities (CSPPPD).
