JOB DESCRIPTION
Three Rivers Center for Independent Living (TRCIL) Services, Inc.
(CSPPPD)
ENROLLMENT ASSISTANT
DUTIES:
1. Complete Eligibility Prescreening for applicants who have been referred for OBRA, Community OBRA, OSP/Independence and Commcare Waiver Services.
2. Maintain working relationships with applicant’s physicians to obtain an MA-51 to determine medical eligibility.
3. Maintain working relationships with the County Assistance Offices throughout TRCIL’s service area to determine financial eligibility.
4. Serves as intake and referral contact for CSPPPD program by phone and in the community.
5. Initializes enrollment process for all referrals.
6. Finalize designated reports to our Administrative Entity and the Department of Public Welfare’s Office of Social Programs and others as assigned.
7. Prepares a separate case record for each applicant once enrolled.
8. Prepares denials for ineligible consumers for enrollment specialists.
9. Interact with applicants in a professional manner with respect for their rights and maintains strict adherence to the independent living philosophy.
10. Attend and participate in general TRCIL staff meetings.
11. Work closely with TRCIL staff to ensure effective and smooth transition into a particular waiver.
12. Provide Individual and Systems Advocacy on issues that will promote the independence, integration and full community participation of persons with disabilities.
13. Represent TRCIL to the community at large.
14. Reports to the CSPPPD Service Coordinator Supervisor.
15. Other duties as assigned.
16. This is a professional, exempt position.
QUALIFICATIONS:
1. Bachelor’s or Associate’s Degree in psychology, sociology, counseling, education or a related field preferred. High School Diploma or GED required.
2. Thorough knowledge of all CSPPPD waiver services.
3. Personal experience with a disability preferred.
4. Ability to work comfortably with people who have severe disabilities.
5. Willingness to travel throughout Western Pennsylvania.
6. Knowledge of and commitment to the independent living philosophy.
7. Ability to work closely with others as part of the application process.
8. Excellent organizational skills.
9. Excellent written and communication skills.
10. Ability to work well under the pressure of deadlines.
11. Personal means of transportation required.
