Community Services Program for Persons with Physical Disabilities

CSPPPD

     The Community Service Program for Persons with Physical Disabilities (CSPPPD) has been administered by TRCIL Services in 23 counties of the western region of the Commonwealth since its inception in 1988. In addition to the main office for TRCIL Services in Wilkinsburg, the program is administered from three satellite offices in Washington, Erie, and New Castle, PA, with a subcontract with Life and Independence For Today (LIFT) in St. Mary's PA.

     The CSPPPD was created by the PA Department of Public Welfare (DPW) to identify individuals with primarily physical disabilities, that manifested before the age of 22, who are residing in or applying for nursing facility care. The goal is to offer, and then coordinate, community based services as an alternative to nursing homes. CSPPPD offers the following options:

  • An Intermediate Care Facility for Persons with Other Relation Conditions (ICF/ORC) - Small 7 bed residential programs which provide intensive skills training prior to moving on to community living. Residents must participate in a daily program designed to meet their goals and needs.

  • A Clustered Shared Living Arrangement (CSLA) - Two or more individuals living in their own homes or individual apartments which are "clustered" in close proximity with each other in order to share an on-call personal assistant during night time hours.

  • An individual apartment or family home.

     The CSPPPD has evolved and expanded over the years and now includes Specialized Services to eligible individuals residing in nursing facilities regardless of the age of onset of disability. Community based services are available to Medicaid eligible individuals regardless of living arrangement at time of application through the OBRA, Independence, Community OBRA, and COMMCare Waivers.

Nursing Facility Services

     Specialized Services are provided to residents with physical disabilities to prepare them to transition to community living if they so desire, or to become more independent and integrated into the community even if they decide to stay in the nursing facility. The services each consumer is eligible for are service coordination, advocacy, skills training, peer support, community integration, transportation, basic education, and transitioning support and coordination.

Community Based Services

     Community Based Services are available to eligible individuals through the various Medicaid Waiver programs described below.

  • OBRA Waiver
    "Targeted" consumers, i.e. those whose disability occurred before the age of 22 who are residing in nursing facilities, are able to relocate to the community and receive home and community based services though this Waiver. The services available are: service coordination, daily living/home support, advocacy, skills training, peer support, community integration, day programming, daily living educational services, personal emergency response installation and maintenance, home modifications, assistive technology, transportation assistance, visiting nurse services, therapies, and respite.

  • Community OBRA Waiver
    Everything is the same as the OBRA Waiver however, the consumer need not be residing in a nursing facility at the time of application.

  • Independence Waiver
    The age of onset of disability has been eliminated for this waiver. The consumer must have a physical disability that has impacted at least 3 major life functions and need more services and assistance than can be provided in the Attendant Care Program. These consumers are most often living in the community in their own home or apartment, or with their families. They are entitled to receive the same level of service as in the OBRA Waiver with the exception of any type of educational services.

  • COMMCare Waiver
    The newest of the Medicaid Waiver programs, is designed to serve individuals,21 years of age or older, with traumatic brain injuries. Eligible individuals can receive many of the same services available in the waivers described above, as well as, coaching and cueing, night supervision, supported employment, prevocational, and educational services.
